Name of victim
Name of bully
Specific school attended
Where incident took place, (i.e. school bus, playground, gym, basketball game, etc.)
When incident took place, (i.e. between 3rd & 4th period, during lunch, on morning bus ride, etc.)
A general description of the incident
Contact information of the person reporting
An investigation will take place and you may be contacted for verification or to provide additional information.
